Save and Open a Form in Acrobat Reader (DC)

Before completing the form, download the form (PDF format) to a drive or location on your computer (e.g., Local Disk (C:), My Documents folder).

• To download the form, click on the link to open the form in your web browser. In the browser's Menu bar, choose File and then choose Save As or Save Page As. In some browsers, you can right-click on the form and choose Save As. You will then be prompted to choose a location on your computer and name the file.
• To open the file, double-click on the file name which will open with Acrobat Reader (DC), and you will be ready to fill out the form.
